Job Title: Diabetes Specialist Nurse

About the Role

This is an exciting opportunity to work as a senior diabetes specialist nurse, providing expert care and support to patients with diabetes. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient-centered care, empowering individuals to manage their condition effectively.

Key Responsibilities
  • Provide advanced assessment and management of patients with diabetes, including education and support to promote self-management.
  • Work collaboratively with other healthcare professionals to develop and implement comprehensive care plans.
  • Conduct regular reviews and assessments to ensure patients are receiving optimal care and support.
  • Develop and deliver educational programs to promote diabetes awareness and self-management skills.
  • Collaborate with other teams to ensure seamless care transitions and coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Registered Nurse Level 1
  • Degree module in Diabetes management or equivalent relevant experience
  • Non-Medical Prescribing Qualification (willing to undertake)
  • Car driver/car user essential
  • Ability to develop colleagues and effectively impart knowledge to others
