Job Title: Diabetes Specialist Nurse

About the Role

This is an exciting opportunity for a motivated and experienced nurse to work in a dynamic and supportive environment. As a Diabetes Specialist Nurse, you will be responsible for:

  • Providing expert advice and support to patients, families, and healthcare professionals on diabetes management and care.
  • Developing and implementing individualized care plans to meet the unique needs of patients with diabetes.
  • Collaborating with other healthcare professionals to ensure seamless care and coordination.
  • Providing education and training to patients, families, and healthcare professionals on diabetes management and self-care.
  • Participating in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements

To be successful in this role, you will need:

  • Registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse.
  • Experience in diabetes care and management.
  • Non-medical prescribing qualification (or willingness to undertake).
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
