Job Summary

We are seeking an experienced Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our diabetes service, you will be responsible for delivering high-quality patient care and education to individuals with diabetes.

Main Responsibilities
  • Assess patient needs and develop personalized care plans in collaboration with the multidisciplinary team.
  • Provide expert advice and support to patients, their families, and healthcare professionals on diabetes management and self-care.
  • Develop and implement structured educational programs for patients and their families, promoting independence in self-management.
  • Work closely with other healthcare professionals to develop a holistic support network for people with diabetes within the Trust, Specialist Units, and Primary Care.
  • Participate in shared responsibility for stock management and reordering as necessary.
  • Act as an autonomous practitioner, providing expert and advanced specialist advice and support to patients, their families, and other healthcare professionals.

Requirements
  • Registered Nurse Level 1
  • Degree module in Diabetes management or equivalent relevant experience
  • Non-Medical Prescribing Qualification (willing to undertake)
  • Car driver/car user essential
  • Ability to develop colleagues and effectively impart knowledge to others
