About the Role

We are seeking an experienced Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our diabetes service, you will be responsible for delivering high-quality patient care and education to individuals with diabetes.

Key Responsibilities

  • Provide advanced assessment and management of patients with diabetes, in line with National Service Framework and NICE guidelines.
  • Develop and implement structured educational programmes for patients and their families, promoting independence in self-management.
  • Work collaboratively with other healthcare professionals to develop a holistic support network for people with diabetes.
  • Act as an expert educational resource and support to all grades of staff in diabetes service, GP Practices and Community Nurses.

Requirements

  • Registered Nurse Level 1
  • Non-Medical Prescribing Qualification
  • Car driver/car user essential
