Diabetes Service Lead

We are seeking an experienced and passionate Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a Senior Diabetes Specialist Nurse, you will play a key role in delivering high-quality patient-centered care to individuals living with diabetes.
Key Responsibilities- Patient Assessment and Care: Conduct comprehensive assessments of patient needs and provide expert advice and support to patients, their families, and other healthcare professionals.
- Professional Development: Maintain and update your own knowledge and skills, and provide evidence to support this, in line with National Service Framework (NSF) and National Institute for Health and Care Excellence (NICE) guidelines.
- Education and Training: Develop and implement structured educational programs for patients and their families and carers, and provide expert educational support to all grades of staff in diabetes service, GP Practices, and Community Nurses.
- Leadership and Governance: Act as an expert educational resource and support to all grades of staff, and participate in shared responsibility for the stock held within the department and reorder as appropriate.
- Autonomy and Expertise: Work clinically for a minimum of 80% of the role, and provide expert and advanced specialist advice and support to patients, their families, and other healthcare professionals.
- Possess the ability to perform assessments of patient needs and liaise with other members of the multi-disciplinary team where necessary.
- Maintain own professional and clinical development in line with standards identified in the NMC Code and by networking with other healthcare professionals at local, national, and international levels.
- Keep up to date with current developments in diabetes and changes within the NHS relevant to community care.
- Assist with developing and implementing structured educational programs for patients and their families and carers.
- As a nurse prescriber, demonstrate advanced knowledge of and experience in the use of Oral Hypoglycemic Agents (OHA's), insulin, and other treatment/technologies regimens relating to diabetes.
