About the Role

We are seeking an experienced and skilled Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our diabetes services.

Key Responsibilities
  • Provide expert and advanced specialist advice and support to patients, their families, and other healthcare professionals.
  • Work clinically for a minimum of 80% of the role, delivering face-to-face consultations and providing evidence-based proactive nursing services.
  • Act as an autonomous practitioner, making decisions and taking ownership of patient care.
  • Develop and implement structured educational programs for patients and their families, using a range of communication skills to engage and empower individuals.
  • Participate in multidisciplinary panel discussions and service development, contributing to the improvement of diabetes care and services.
  • Initiate and continue assessment of insulin pump therapy, interpreting results and initiating appropriate action.
  • Maintain clear, concise, and accurate patient records, using electronic systems to document patient information.
Requirements
  • NMC Registered
  • Evidence of experience in diabetes management
  • Evidence of further study to masters level in diabetes
  • Evidence of experience in the use of diabetes technology
  • Access to car/car driver
Desirable Qualifications
  • Non-medical prescriber or willing to work towards
  • Competent in the use of information technology (EMIS, CERNER, ICE)
  • Evidence of working as part of the multidisciplinary team
