About the Role

We are seeking an experienced Paediatric Diabetes Nurse Specialist to join our dynamic team at East Lancashire Hospitals NHS Trust. As a key member of our Paediatric Diabetes Service, you will play a vital role in providing high-quality care to children and young people with diabetes.

Key Responsibilities
  • Manage and develop the Children's Diabetes Service to facilitate early discharge and prevent avoidable re-admission to hospital.
  • Provide highly specialized clinical advice and support to healthcare professionals and others.
  • Support the Education Lead nurse in planning, preparing, and delivering diabetes education to children and young people and their families.
  • Build strong links with nurseries, schools, and colleges to support teaching and managing the specific health needs of children and young people with diabetes.
  • Support families in managing their child's diabetes regime at home and in the community.
  • Provide targeted support for children and young people with sub-optimum Hba1c levels.
  • Jointly lead the team alongside other Band 7s and support other members of the team.
  • Identify families with high-risk indicators and follow trust policies regarding safeguarding children and young people.
  • Ensure up-to-date documentation on electronic systems and undertake training as required.

Requirements
  • 2 years' experience in Paediatric Diabetes nursing.
