Job Summary

We are seeking an experienced and passionate Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient-centered care to individuals living with diabetes.

Key Responsibilities
  1. Patient Assessment and Care: Conduct comprehensive assessments of patient needs and develop personalized care plans in collaboration with the multidisciplinary team.
  2. Education and Support: Provide education and support to patients, their families, and carers to empower them to manage their diabetes effectively.
  3. Clinical Governance and Risk Management: Demonstrate a sound understanding of Clinical Governance and Risk Management principles and apply them to the work situation.
  4. Leadership and Collaboration: Act as an expert educational resource and support to all grades of staff in diabetes service, GP Practices, and Community Nurses in the delivery of comprehensive diabetes care.
  5. Professional Development: Maintain own professional and clinical development in line with standards identified in the NMC Code and by networking with other healthcare professionals at local, national, and international levels.
Requirements
  • Registered Nurse with a valid NMC registration
  • Postgraduate qualification in Diabetes Specialist Nursing or equivalent
  • Experience in diabetes care and management
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
