About the Role

We are seeking an experienced and skilled Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and supporting individuals with diabetes to manage their condition effectively.

Key Responsibilities
  • Provide advanced assessment and advice on diabetes management to patients, their families, and healthcare professionals.
  • Develop and implement structured educational programs for patients and their families, promoting independence in self-management.
  • Work collaboratively with other healthcare professionals to develop a holistic support network for individuals with diabetes.
  • Participate in shared responsibility for stock management and reordering as necessary.
  • Provide evidence-based proactive nursing services, responsive to user needs, and delivered through face-to-face consultation.
  • Act as an autonomous practitioner, providing expert and advanced specialist advice and support.

Requirements
  • Registered Nurse Level 1
  • Degree module in Diabetes management or equivalent relevant experience
  • Non-Medical Prescribing Qualification (willing to undertake)
  • Car driver/car user essential
  • Ability to develop colleagues and effectively impart knowledge to others
