Position Overview

The individual in this role will play a crucial part in facilitating the effective delivery of clinical services by actively assisting multidisciplinary teams (MDT) and service transition teams (STT) in managing care pathways for patients diagnosed with cancer.

Key Responsibilities

This position serves as the primary contact for providing essential information and assistance to the MDT at critical junctures of the patient's journey, ensuring that individuals can effectively navigate the healthcare and social support systems. The objective of this role is to guarantee that cancer patients receive appropriate support in a timely manner. Responsibilities include:

  • Coordinating services and enhancing communication among caregivers, treatment providers, and families to create a cohesive patient experience.
  • Acting as a single point of access for cancer patients, facilitating rapid re-entry into the system for those with urgent or specialized needs.
  • Complementing existing STT teams and supporting patients throughout their diagnostic and treatment pathways, including follow-up care.

Role Responsibilities

The scope of this role may vary based on the focus areas of Lower GI, Upper GI, and Hepatobiliary services, adapting to the priorities established during the role's development. Core responsibilities include:

  • Serving as a point of contact for patients on a 2-week wait pathway diagnosed with cancer, ensuring effective communication with the clinical team.
  • Coordinating appointments, tracking results, and addressing queries related to diagnostic processes.
  • Providing guidance to support services and collaborating with the MDT to ensure a patient-centered approach.

  • Upholding a positive image of the Trust while ensuring compliance with policies and procedures.
  • Participating in relevant meetings and contributing expertise in specialized areas.
  • Supporting resource management and maintaining a safe working environment.
