Position Overview

We are seeking a dedicated and skilled Physician to join our community healthcare team on a part-time basis.

The Community Healthcare Physician will operate independently and collaborate with Advanced Clinical Practitioners to deliver medical services to an inpatient population, including beds dedicated to supporting rehabilitation pathways.

The Physician will fulfill a medical role, focusing on assessing, planning, and managing medical risks to assist service users and the broader multidisciplinary team (MDT).

When required, the incumbent will partner with ACPs or trainee ACPs to identify and address the needs of the ward and patients, manage medical complexities, and provide training and direct clinical oversight.

Comprehensive induction will be provided.

The role will be conducted in accordance with the GMC's professional conduct guidelines and the local policies and procedures of Gloucestershire Health and Care NHS Foundation Trust.

This position will enhance collaborative working with nursing staff, medical professionals, and allied health practitioners through effective role modeling, managing medical complexities, and offering a comprehensive skill set to facilitate ongoing biopsychosocial assessments of service users.

The incumbent will support initiatives aimed at monitoring and enhancing standards through supervision, evidence-based practices, clinical audits, and research activities.

Key Responsibilities

Deliver high-level assessments and diagnoses for patients presenting with a variety of clinical conditions, while developing a foundational understanding of palliative and geriatric care. Exhibit extensive knowledge regarding disease patterns, indicators of condition progression, and treatment options available at various stages of illness. Utilize professional judgment to appropriately manage risks, particularly in complex and unpredictable situations, while guiding teams to ensure the safety of individuals, families, and caregivers. Collaborate with MDT colleagues to ensure optimal ongoing care for service users, including effective discharge planning and follow-up care. Facilitate timely and effective Advanced Care Planning/ReSPECT discussions within the MDT and with service users. Work alongside ward colleagues to anticipate and address the needs of patients entering end-of-life phases, including parallel planning where necessary, and providing effective palliative support and care during the final days of life. Establish and maintain effective communication with various stakeholders, managing any barriers that may arise. Engage in clinical supervision to support colleagues in reflecting on their practice, enhancing knowledge, and improving the quality of care provided to service users.
