About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our Community Nursing Service in Stroud Locality. As a key member of our Integrated Community Team, you will play a vital role in delivering high-quality patient care in the community.

Key Responsibilities
  • Participate in the delivery of care to patients and service users as part of a Mental Health and Social Care Health Service.
  • Assist in the assessment, planning, and implementation of care plans to meet the needs of patients and service users.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support.
  • Develop and maintain effective relationships with patients, families, and carers to ensure person-centered care.
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes.
Requirements
  • Minimum of 5 GCSEs (or equivalent) including English, Mathematics, and a Science or Health and Social Care based subject.
  • NVQ Level 3 in Care Certificate.
  • Experience of direct adult patient care in a Health or Social Care setting.
  • Relevant clinical competencies, such as venepuncture, catheter care, and personal care.
  • Evidence of continuous personal development.
