Position Overview
We are seeking a dedicated and compassionate Community Nurse specializing in Haemoglobinopathies to join our team. This role is integral to enhancing the quality of care for individuals affected by sickle cell disease and thalassaemia within our community.
Key Responsibilities
The successful candidate will collaborate with multidisciplinary teams to ensure the delivery of exceptional haemoglobinopathy services. Responsibilities include:
- Providing comprehensive health education to empower patients and their families in managing their conditions.
- Promoting adherence to preventive measures, including vaccinations and medication regimens.
- Addressing lifestyle and social determinants that affect health outcomes, while connecting patients to local resources.
- Building strong, trusting relationships with patients to facilitate effective care.
- Offering targeted support to frequent users of healthcare services, guiding them towards appropriate programs.
- Delivering in-reach services to secondary care facilities as needed.
- Working closely with healthcare professionals across various settings to ensure continuity of care.
- Implementing personalized care plans tailored to individual patient needs.

Qualifications and Skills
Applicants should possess:
- A valid registration as an Adult Nurse with the Nursing and Midwifery Council (NMC).

- Experience in multidisciplinary teamwork and a commitment to patient advocacy.
