About the Role

We are seeking a highly skilled and dedicated Community Haemoglobinopathy Nurse to join our team at North Middlesex University Hospital NHS Trust. As a key member of our haemoglobinopathy service, you will play a vital role in providing high-quality care to patients with sickle cell disease and thalassaemia.

Key Responsibilities
  • Provide comprehensive health education and guidance to patients and their families, empowering them to manage their conditions effectively.
  • Enhance compliance with preventative actions, such as vaccinations and prophylactic medications, to reduce the risk of acute episodes and hospital admissions.
  • Address social and lifestyle factors that impact condition management and overall quality of life, directing patients to appropriate local resources and support services.
  • Build trust with patients and their families, providing individualized care and support to meet their unique needs.
  • Work closely with multidisciplinary colleagues in primary, secondary, and social care to ensure seamless continuity of care for patients.
  • Contribute to the development and implementation of individual care plans, ensuring that patients receive tailored support and interventions.
  • Provide in-reach services to secondary care, facilitating early discharge and reducing hospital readmissions.
  • Collaborate with haematologists, ward sisters, and nurses to initiate timely early discharge planning and facilitate community services for early discharges.
  • Support nurse-led clinics, telephone, or face-to-face, interpreting results, taking appropriate action, and feeding back into multidisciplinary team meetings and reviews.

Requirements
  • First-level registered adult nurse with the NMC
  • Relevant educational/training qualification/certificate/experience
  • Ability to manage competing demands and priorities in a changing, complex environment
  • Phlebotomy skills
  • Competent IT skills, including Microsoft Office apps
  • Post-registration nursing experience
  • Knowledge of sickle cell disease and thalassaemia
  • Experience of caseload management
  • Evidence of multidisciplinary and multiagency working
  • Ability to work under own initiative with good organisational skills
  • Self-motivated and autonomous, with a commitment to team working
