Job Overview

An exceptional opportunity has arisen in our department to work as a leading expert in the field of sickle cell and haemoglobinopathies. As a Transition Nurse Specialist, you will have the opportunity to manage a caseload of patients/clients through the process of transition from Paediatric to Adult Haematology Services.

Main Responsibilities
  • Work as an expert practitioner in the field of sickle cell and haemoglobinopathies, effectively coordinating and delivering an expert nursing service for patients aged 14-24 years with a diagnosis of Sickle Cell and Thalassaemia.
  • Manage a caseload of patients/clients through the process of transition from Paediatric to Adult Haematology Services.
  • Ensure that patients and their families are fully informed and supported by providing clinical and social support, including the running of nurse-led clinics.
  • Lead in the development of evidence-based clinical nursing practice in line with current research and guidelines.
  • Teach/train healthcare teams at the Trust around transition and haemoglobinopathies.
  • Provide line management to the adult Haemoglobinopathies Clinical Nurse Specialist team and HCC Practice development Nurse.
  • Liaise with key stakeholders to provide an annual audit report and case stories, attend conferences and workshops, mentor other specialist nurses, and collaborate on service improvement and innovation projects in the area of transition and haemoglobinopathies.
  • Oversee the caseload for adult service.
Requirements
  • NMC Level 1 or Level 15 Registered Nurse.
  • Master's Degree or working towards.
  • Relevant, demonstrable experience in the care of patients with haemoglobinopathies.
  • Non-Medical Prescribing.

  • Excellent communication/counselling skills.
