Main Responsibilities

The Community Nurse will work closely with haemoglobinopathy teams to ensure the effective delivery of care. Key responsibilities include:

  • Providing health education and self-management guidance to minimize the risk of acute episodes and reduce hospital admissions.
  • Promoting adherence to preventive measures, including vaccinations and prophylactic treatments.
  • Addressing social determinants of health that affect condition management and overall quality of life, while connecting patients to local resources.
  • Building trusting relationships with patients and their families.
  • Offering targeted support to frequent users of secondary care services, including facilitating access to existing programs.
  • Delivering in-reach services to secondary care settings.
  • Collaborating with colleagues across primary, secondary, and social care to ensure continuity of care.
  • Implementing individualized care plans for patients in various settings.

Detailed Job Description and Responsibilities

  • Empower patients and caregivers through health education, enabling informed decisions regarding their care.
  • Enhance compliance with preventive health measures, including vaccinations and medications.
  • Guide patients in self-management strategies to prevent exacerbations of their conditions.
  • Address lifestyle factors impacting health and connect patients with appropriate support services.
  • Act as an advocate for patients, applying ethical and professional knowledge to meet diverse healthcare needs.
  • Provide focused support to high-intensity users of healthcare services, facilitating access to relevant programs.
  • Engage with patients prior to appointments to reduce non-attendance rates.
  • Implement individualized care plans across all settings.
  • Advise on medication management at home, including dosages and non-pharmacological techniques, with regular follow-ups to monitor effectiveness.
  • Utilize advanced prescribing skills as appropriate.
  • Deliver in-reach services to secondary care facilities.
  • Organize and facilitate patient-led education sessions and outreach programs.
  • Promote best practices in line with evidence-based guidelines.
  • Provide timely nursing interventions, seeking senior support when necessary.
  • Recognize when a patient's needs exceed personal clinical capabilities and consult with senior team members.
  • Contribute to a multidisciplinary approach for managing patients referred to community services.
  • Collaborate with haematology teams to facilitate timely discharge planning and community service access.
  • Offer personalized advice and support to patients and their families to prepare for discharge.
  • Support nurse-led clinics, interpreting results and contributing to multidisciplinary team discussions.

Education and Qualifications

Essential Criteria

  • Registered Adult Nurse with the Nursing and Midwifery Council (NMC).
  • Relevant qualifications and experience in nursing.

Desirable Criteria

  • Qualified non-medical prescriber.

Skills and Abilities

  • Phlebotomy skills.
  • Proficient in IT, including Microsoft Office applications.
