Adult Haematology Service Coordinator

We are seeking an experienced and skilled nurse to join our team as a Sickle Cell and Thalassaemia Specialist Nurse. This is an exciting opportunity to work with patients aged 14-24 years with a diagnosis of Sickle Cell and Thalassaemia, and to coordinate and deliver an expert nursing service.
Main Responsibilities- Expert Practitioner: Work as an expert practitioner in the field of sickle cell and thalassaemia, and effectively coordinate and deliver an expert nursing service for patients aged 14-24 years.
- Caseload Management: Manage a caseload of patients/clients through the process of transition from Paediatric to Adult Haematology Services.
- Patient Support: Ensure that patients and their families are fully informed and supported by providing clinical and social support, including the running of nurse-led clinics.
- Leadership and Development: Lead in the development of evidence-based clinical nursing practice in line with current research and guidelines, and teach/train healthcare teams at the Trust around transition and haemoglobinopathies.
- Line Management: Provide line management to the adult Haemoglobinopathies Clinical Nurse Specialist team and HCC Practice development Nurse.
- Collaboration and Liaison: Liaise with Roald Dahl's Marvellous Children's Charity, providing an annual audit report and case stories, attending their conferences and workshops, and mentoring other Roald Dahl Specialist Nurses.
- Caseload Oversight: Oversee the caseload for adult service.
- Education and Qualifications: NMC Level 1 or Level 15 Registered Nurse, expert practitioner either through formal qualification (Degree) and extensive clinical experience pertinent to the area, Master's Degree or working towards.
- Knowledge and Experience: Experience of working in paediatric services and ongoing interest in the care of young people, proven experience in service development, experience in delivering presentations, experience in running research and audit programmes.
- Skills and Competencies: Excellent communication/counselling skills, strong leadership skills, advanced clinical assessment skills or willingness to develop these, apheresis skills.
