Position Overview

The Alcohol & Drug Service is seeking a dedicated Substance Misuse Support Coordinator to join our innovative team. This role is pivotal in providing comprehensive support to individuals facing challenges related to substance misuse.

Key Responsibilities

  • Engage with individuals experiencing substance misuse issues in various outreach environments, facilitating access to treatment services and relevant community resources.
  • Conduct thorough assessments to determine the eligibility, strengths, and needs of service users, ensuring tailored support.
  • Implement risk management and care plans in accordance with established policies and procedures.
  • Deliver individualized care packages, incorporating psychosocial and psychological interventions as necessary.
  • Collaborate with health and social care professionals to ensure a holistic approach to service delivery.
  • Participate actively in multi-disciplinary team meetings, contributing to the overall care planning process.
  • Communicate sensitive information effectively within challenging environments, maintaining professionalism at all times.
  • Assist in the evaluation of service users' needs, particularly in relation to safeguarding and welfare.
  • Maintain accurate and up-to-date records of service user interactions and progress.
  • Promote the service and its offerings during internal and external engagements.

Qualifications and Experience

  • A relevant degree in Social Work, Psychology, or a related field, or equivalent qualifications in health and social care.
  • Demonstrable experience in the substance misuse sector, particularly in outreach or complex needs settings.
  • Strong understanding of the recovery movement and the complexities involved in multi-disciplinary collaboration.
  • Proficient in report writing and conducting assessments.
  • Ability to work autonomously while effectively managing time and resources.
