Reablement Physiotherapist Opportunity

At Hunter AHP, we are seeking a skilled Physiotherapist to join our team in a Reablement and Community Rehabilitation role. This position involves providing therapy assessment and early intervention to patients under social care and Reablement services, with the goal of preventing unnecessary hospital admissions due to functional deterioration and exacerbation of long-term conditions.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical abilities and develop personalized rehabilitation plans
  • Provide early intervention and treatment to prevent hospital admissions and promote independence
  • Collaborate with multidisciplinary teams, including Therapy Assistants, Physiotherapists, and Occupational Therapists, to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions

Requirements:

  • Experience in Reablement and Community Rehabilitation
  • Strong assessment and treatment skills
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
