An exceptional opportunity has emerged for a highly skilled Occupational Therapist/Physiotherapist to join our Rehabilitation team, which operates within a collaborative community service framework.

Key Responsibilities

This position offers a wealth of learning and professional development opportunities. You will collaborate with a dynamic multidisciplinary team (MDT) that prioritizes high-quality, patient-centered care. Our goal is to deliver exceptional, client-focused therapy to individuals in the community.

Your caseload will encompass both community settings and transitional care for patients in nursing facilities, supported by the MDT. You will primarily work with adults who may have multiple complex health conditions, allowing you to enhance your clinical skills in various areas, including discharge planning, falls prevention, manual handling, orthopaedics, and general medical and surgical care. Familiarity with managing patients with multiple health issues and understanding the influence of psychological and cognitive factors is advantageous.

This role will involve working across various care settings, with a designated office base for administrative tasks.

Job Duties

You will conduct comprehensive assessments and interventions, making high-stakes clinical decisions. Working autonomously, you will also collaborate with colleagues in primary and secondary care, utilizing evidence-based practices and specialized skills to assess, plan, implement, and evaluate interventions, ensuring that all team members adhere to these standards.

As a lead therapist within the team, you will provide supervision and guidance to junior therapists and students, ensuring the delivery of evidence-based rehabilitation. You will manage your own caseload, conducting individual assessments and home visits for patients in their residences and care facilities. Additionally, you will take the lead on manual handling, pressure care, and equipment requirements within the team, supporting the team lead as necessary.

Qualifications and Experience
