Position Overview

An exceptional opportunity has emerged to become a vital member of the Central London Community Health Trust's Discharge to Assess Team. We are seeking skilled professionals, including administrators, rehabilitation assistants, and physiotherapists, to collaborate closely with colleagues in Adult Social Care to provide outstanding reablement services to our community.

This role is a fixed-term contract with the potential for extension, offering a unique chance to engage in integrated care delivery within a supportive framework, ensuring high-quality, patient-centered services.

Key Responsibilities

As a Lead Community Physiotherapist, your primary duties will include:

  • Delivering specialized physiotherapy services to a diverse community caseload.
  • Conducting thorough assessments of patients referred to the team to guarantee their safety and appropriate care pathways.
  • Providing clinical guidance and mentorship to the Allied Health Professional (AHP) team, facilitating education and supervision for junior staff, rehabilitation support workers, and students.
  • Assisting and stepping in for the operational manager of the service as needed.
  • Leading initiatives for service enhancement, developing managerial competencies, and spearheading audit and research projects.

Qualifications and Experience

Essential:

  • Degree or diploma in physiotherapy.
  • Current registration with the Health Care Professions Council.
  • Demonstrated ongoing professional development through a CPD portfolio.
  • Participation in specialized training or equivalent CPD activities in Adult Rehabilitation or geriatric care.

Desirable:

  • Membership in the Chartered Society of Physiotherapy.
  • Postgraduate qualifications in a relevant field.
  • Involvement in a Special Interest Group pertinent to this role.

Experience Requirements

Essential:

  • Experience at a senior level across various clinical settings, including neurology, general rehabilitation, and elderly care.

Desirable:

  • Experience with specialized services such as falls prevention, wheelchair services, and care for individuals with learning disabilities.
  • Prior experience as a Band 6 Physiotherapist.
  • Experience in community rehabilitation settings.
  • Background in the National Health Service (NHS).

Skills and Knowledge

Essential:

  • Proficient in assessing and treating adults with complex therapy needs.
  • Understanding of intricate medical conditions, including neurology and multi-pathologies.
  • Knowledge of the roles of Allied Health Professionals and interdisciplinary collaboration.
  • Ability to apply core physiotherapy skills across various clinical conditions with a client-centered approach.
  • Familiarity with outcome measures and evidence-based practices relevant to community physiotherapy.
  • Awareness of national guidelines and frameworks related to falls and long-term conditions.
  • Strong problem-solving abilities and clinical reasoning skills.
  • Excellent communication and documentation skills, both verbal and written.
  • Experience in teaching and mentoring junior staff and students.
  • Understanding of the roles of social services and voluntary organizations.

Desirable:

  • Experience supervising assistant and junior staff.
  • Knowledge and experience in research and audit processes.
  • Familiarity with national frameworks and guidelines for older populations and progressive conditions.
