Position Overview

Central London Community Health Trust - Our organization is dedicated to providing a variety of community health services, and we are currently looking for a motivated Specialist Physiotherapist to become part of our Unplanned Care team.

This role is based in a community setting, focusing on delivering care to patients in their homes, thereby reducing the need for hospital admissions in the local area.

The Unplanned Care team operates through five distinct pathways, which include: Discharge to Assess Pathway (DTA), Rapid Response (RR), Frailty Hospital at Home/virtual ward, Early Intervention Vehicle (EIV), and Prevention to Admission. The primary responsibilities will involve working within the Discharge to Assess and Rapid Response pathways, with the potential to engage in other pathways as necessary. All pathways are designed to provide nursing and therapeutic care in the comfort of the patient's home.

Key Responsibilities

The Rapid Response pathway aims to avert unnecessary hospital admissions for patients experiencing acute health issues by conducting comprehensive assessments within two hours of referral and providing treatment over a five-day period.

The Discharge to Assess pathway facilitates the safe transition of patients from acute and community hospitals, ensuring that rehabilitation and therapy needs are met to support a secure discharge when the patient is medically stable.

The Early Intervention Vehicle service addresses referrals for patients identified as needing assistance for low-complexity health and functional challenges that could otherwise lead to hospital admissions due to loss of independence or falls.

Our Unplanned Care team collaborates closely with other community service providers to ensure comprehensive support is available.

The team operates across various localities, providing clinical support at acute hospitals to prevent admissions and promote timely discharges.

Job Responsibilities

  • The ideal candidate will be a registered physiotherapist with advanced clinical skills and substantial experience in the care of adults and older individuals across various healthcare environments.
  • Candidates should demonstrate self-motivation, strong organizational abilities, and the capacity to make independent clinical decisions.
  • As a clinician, you will utilize your expertise, judgment, and decision-making skills to support patients at home, enhance patient outcomes, and minimize the risk of unnecessary hospital admissions.
  • You will conduct comprehensive assessments, provide appropriate interventions, and possess knowledge of community services to plan ongoing patient care.

Required Qualifications

  • Diploma or degree in Physiotherapy.
  • Current registration with the relevant professional body.
  • Evidence of ongoing professional development.
  • Participation in appraisal schemes.
  • Attendance at specialized courses or equivalent professional development activities in areas such as Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, and Geriatric Rehabilitation.
  • Membership in a professional organization.

Experience and Skills

  • Essential experience as a Band 5 physiotherapist.
  • Experience in various specialized clinical areas, including outpatient services, neurology, rehabilitation, orthopedics, and respiratory care.
  • Skills in assessing and treating adults with complex health conditions.
  • Knowledge of interdisciplinary team dynamics and the role of Allied Health Professionals.
  • Strong communication and report writing skills.
  • Basic IT proficiency, including Microsoft Office applications.

Additional Requirements

  • A valid UK driver's license and access to a vehicle.
  • Ability to work collaboratively within a team.
  • Flexibility and adaptability in a dynamic work environment.
  • Capacity to manage pressure and resolve conflicts effectively.
