**Job Summary**
Six Physio Ltd is seeking a highly skilled and experienced Senior Community Physiotherapist to join our team. As a key member of our team, you will be responsible for providing high-quality physiotherapy services to patients in their community and within residential care homes across Central, South-West and North-West London.
**Key Responsibilities**
- Conduct thorough assessments of patients' medical history and relevant operation notes
- Develop and implement effective rehabilitation plans tailored to individual patients' needs
- Utilize in-depth knowledge of neurological and orthopaedic conditions and treatments, as well as aged care and rehabilitation principles
- Apply orthopaedic skills and knowledge to provide comprehensive care
- Experience with hydrotherapy is highly desirable
- Work effectively with patients with dementia and the elderly
**Requirements**
- Bachelor of Science or equivalent degree in Physiotherapy
- Proven experience of lone working and managing travel between patients
- Up-to-date DBS certificate is preferred
