Adults Community Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our team at Central London Community Healthcare NHS Trust. As a Physiotherapist, you will play a vital role in delivering high-quality care to adults in the community, promoting independence and improving health outcomes.
About the RoleThe successful candidate will be responsible for assessing and treating adults with complex multi-pathologies, using evidence-based practice and outcome measures to inform their care. You will work closely with other healthcare professionals to provide holistic support and ensure seamless care transitions.
Key Responsibilities- Conduct comprehensive assessments of adults with complex health needs, identifying areas for intervention and developing personalized care plans.
- Deliver high-quality physiotherapy interventions, including exercise programs, manual therapy, and education, to promote independence and improve health outcomes.
- Collaborate with other healthcare professionals to ensure seamless care transitions and provide holistic support to adults in the community.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve health outcomes.
- Contribute to the development and implementation of physiotherapy services, including service evaluation and quality improvement initiatives.
To be successful in this role, you will need:
A degree in Physiotherapy and current registration with the Health and Care Professions Council (HCPC).Experience working with adults with complex health needs, including those with neurological, musculoskeletal, and respiratory conditions.Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.Ability to work autonomously and as part of a team, with a flexible and adaptable approach to meet the changing needs of patients and services.Basic IT skills, including Microsoft Office and electronic patient record systems.What We OfferWe offer a comprehensive induction program, including training and support to ensure you have the skills and knowledge to excel in this role.
