Job Summary

We are seeking a highly skilled and experienced Physiotherapist to join our team at Central London Community Healthcare NHS Trust. As a Senior Physiotherapist, you will be responsible for delivering high-quality care to patients in their homes, working closely with our multidisciplinary team to prevent unnecessary hospital admissions.

Main Responsibilities:

  • Conduct holistic assessments and develop personalized care plans for patients with complex medical conditions.
  • Deliver evidence-based interventions to improve patient outcomes and reduce the risk of hospital admission.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.

Requirements:

  • Registered Physiotherapist with advanced clinical skills and experience in adult rehabilitation.
  • Strong knowledge of community services and ability to plan ongoing care for patients.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
