About the Role

As part of a dynamic community-based multidisciplinary team, you will conduct assessments and provide equipment recommendations to enhance the well-being and functional abilities of vulnerable adults. Your expertise will contribute directly to improving outcomes for those seeking to live fulfilling lives within their communities.

Key Responsibilities

  • Conduct comprehensive assessments to determine individual needs and develop tailored care plans.
  • Collaborate closely with Occupational Therapists, Social Workers, Physiotherapists, and Enablers to ensure holistic support.
  • Provide expert guidance on the selection and provision of appropriate OT equipment to facilitate independence.
  • Maintain accurate documentation and records in accordance with established protocols.
  • Contribute to a positive and supportive work environment that fosters teamwork and shared goals.

Essential Requirements

  • Proven experience conducting assessments within the Social Services sector.
  • In-depth knowledge of OT equipment and its applications in supporting functional independence.
  • Enhanced DBS clearance.
  • Excellent communication, interpersonal, and problem-solving skills.
  • A commitment to providing compassionate and person-centered care.
