Reablement Physiotherapist Opportunity
Community Rehabilitation Physiotherapist: Hunter AHP has a great locum opportunity for a Physiotherapist with experience in discharging and working in the community to prevent readmission. This is a role in a multi-disciplinary community Rehabilitation Therapy team that is made up of Therapy assistants, Physiotherapists, and Occupational Therapists. In this role, you will be providing therapy assessment for patients under social care and Reablement service and providing early physiotherapy intervention to avoid unnecessary admission due to functional deterioration and exacerbation of long-term conditions.

- Up to £30 per hour
- Part-time or full-time
About the Role:
- Provide therapy assessment and early intervention to patients under social care and Reablement service.
- Work in a multi-disciplinary community Rehabilitation Therapy team.
- Collaborate with Therapy assistants, Physiotherapists, and Occupational Therapists.
- Contribute to the prevention of unnecessary admission due to functional deterioration and exacerbation of long-term conditions.
