Job Description

Job Summary

This role will be based across Intermediate Care Lambeth (Rehab & Reablement), providing home-based support and rehabilitation for clients with a wide range of functional difficulties, long-term conditions, and disabilities. The purpose of this service is to maximize independence and prevent unnecessary hospital admissions, facilitate hospital discharge, and reduce reliance on long-term care where possible.

Main Responsibilities

  • Contribute to the assessment, planning, implementation, and review of individual rehabilitation and care programs for clients, including home exercises and Activities of Daily Living to increase client function and independence.
  • Independently implement rehabilitation and care programs in patients' properties, including teaching prescribed exercise programs, practicing transfers and mobility, and promoting independence in personal care and domestic tasks.
  • Monitor clients' progress, amend programs independently, and report back to relevant professionals or duty therapists as required, following the local escalation procedure.
  • Work as part of a flexible multidisciplinary team, undertaking tasks delegated by Occupational Therapists, Physiotherapists, Nurses, and Social Workers in various settings, primarily in clients' homes or communities.
  • Communicate effectively with clients and their carers, particularly those with impairments affecting their ability to communicate needs and give informed consent to treatment.
  • Support and develop Rehabilitation Support Workers within a competency framework and supervise them with senior staff support.
  • Participate in prioritization and triage of referrals, based on given criteria and departmental protocols, and contribute to clinical governance and service development activities.

Requirements

  • Understanding of a wide range of physical and psychological conditions and their impact on clients' abilities to undertake everyday activities, mobility, and communication.
  • A basic understanding of Occupational Therapy, Speech and Language Therapy, Physiotherapy, and Social Work, and the role of a support worker/Therapy Assistant in supporting the rehabilitation and assessment team.
  • Competent in the use of basic IT, including MS Word, email, internet, and other administrative tasks.
  • Fitness to perform duties requiring a moderate to intense level of physical activity over a sustained period, including travel around the borough and clinical tasks.
  • Able to demonstrate and use planning, organizational, and time management skills to prioritize and organize client caseloads and competing time and workload demands.
  • Ability to communicate and negotiate effectively with clients, carers, and colleagues to achieve the best outcomes for patients.

Desirable Qualifications and Experience

  • Relevant significant experience working in a health, education, or social care setting.
  • Relevant significant experience working with adults and older people with a physical disability for at least one year.
  • Experience of working in a rehabilitation setting, community/lone working, and contributing to service development or training.
