About the Role

We are seeking a highly skilled and compassionate Cancer Care Coordinator to join our team at Self Help Nottingham. As a key member of our Primary Care Network, you will play a vital role in providing holistic support to patients on the cancer pathway.

Key Responsibilities
  • Conduct in-person Cancer Care Reviews and liaise with a range of professionals to address the wider wellbeing needs of patients.
  • Assess and support patients to ensure their needs are met, working closely with the Cancer Nurse Specialist, GP, and other services.
  • Interact with professionals within the PCN to ensure the best cancer journey for each patient.
  • Participate in community outreach and screening initiatives to promote cancer awareness and support.
Requirements
  • Full driving licence and access to a car for work use.
  • Good planning, organisational, and time management skills.
  • Ability to work as part of a multi-disciplinary team and on own initiative.
  • Strong interpersonal skills, including tact, diplomacy, persuasion, and negotiating skills.
  • Ability to communicate effectively in writing and produce documents in a range of formats.
  • Experience of providing support and information in people-facing roles within the health, community, or voluntary sector.
  • Experience of assessing and supporting vulnerable people and those with long-term health conditions.
