Position Overview
The Haematology Care Pathway Coordinator will operate under the guidance of a registered nurse, delivering exceptional, patient-centered support to individuals diagnosed with Haematological Cancer.
This role encompasses facilitating care delivery, managing the patient's journey, evaluating patient and caregiver requirements, and offering comprehensive information and assistance to those affected by Haematological cancer.
As an integral member of the Haematology cancer multidisciplinary team (MDT), the Coordinator will ensure seamless communication among the Trust, tertiary care providers, primary healthcare, and both statutory and voluntary organizations.
Key Responsibilities
- Implement care plans for patients, contributing to adjustments as necessary, including the completion of electronic Holistic Needs Assessments (eHNAs).
- Provide guidance to patients, family members, or healthcare professionals within the designated care area.
- Assist in the advancement of services, protocols, and policies.
- Manage personal resources related to inventory control.
- Maintain accurate patient records, including data entry and processing.
- Stay informed about Trust-held computer systems.
- Participate in clinical and non-clinical audits relevant to the area of care.
- Adhere to Trust standard operating procedures and policies.
- Consult with qualified staff for guidance as needed.

General Duties
All employees at Nottingham University Hospitals NHS Trust are expected to adhere to the following general duties:
- Contribute to holistic needs assessments and the formulation of personalized care plans under the supervision of a registered nurse.
- Implement, monitor, and review care plans with patients and caregivers, promptly reporting any changes in the patient's condition to the registered nurse.
- Evaluate the effectiveness of care delivery in collaboration with the registered nurse.
- Conduct patient reviews on the ward under the direction of the registered nurse.
- Demonstrate the ability to recognize and respond appropriately to sudden deteriorations or emergencies, alerting the team as necessary.
- Triage incoming calls to the service, providing initial assessments to patients or caregivers.
- Address inquiries and determine the most suitable solutions, discussing complex cases with the Lymphoma and CAR-T Clinical Nurse Specialists.
- Offer general telephone support, directing patients and caregivers to additional resources as appropriate.
- Coordinate appointments and investigations to enhance efficiency and patient experience.
- Collaborate with the MDT coordinator to ensure a smooth service for patients and a seamless transition through the cancer care pathway.
- Identify the informational needs of patients and caregivers, connecting them with the Macmillan Cancer Information & Support Centre.
- Recognize indications of changing needs through telephone interactions and respond accordingly.
- Assist the registered nurse in coordinating care to ensure safe and effective transitions between services.
- Support the delivery of education and advice to patients and caregivers.
- Advise patients on self-care management and education for non-complex cases, including self-assessment techniques.
- Encourage and promote healthy lifestyle choices.
- Provide Level 1 psychological support to patients and caregivers, referring those needing Level 2 support to the registered nurse.
- Conduct scheduled follow-up calls to patients to assess their health and well-being, addressing any emerging needs and referring to Nurse specialists as appropriate.
Qualifications and Experience
Essential:
- NVC level 3 or equivalent experience.
- Previous experience in an acute hospital setting.
Desirable:
- Experience with PICC line care.
- Experience with peripheral blood sampling.
Skills and Attributes
Essential:
- Proficient in Microsoft Outlook, Word, and Internet Explorer.
- Ability to work independently and manage emotional situations effectively.
- Strong prioritization skills and the ability to work under pressure.
- Flexible attitude and approach to work.
