Position Overview

As a vital member of the healthcare team, the Haematology Care Pathway Coordinator will work closely with registered nursing staff to deliver exceptional, patient-centered care to individuals diagnosed with Haematological cancers.

This role encompasses the coordination of care delivery, assessment of patient and caregiver requirements, and the provision of comprehensive information and support to those affected by Haematological conditions.

In collaboration with the multidisciplinary team (MDT), the Coordinator will ensure effective communication among various healthcare providers, including tertiary facilities, primary care, and community organizations.

Key Responsibilities

  • Implement and monitor care plans for patients, contributing to adjustments as necessary, including the completion of electronic Holistic Needs Assessments (eHNAs).
  • Provide guidance and support to patients, families, and healthcare professionals within the designated care area.
  • Assist in the development of service protocols and policies to enhance care delivery.
  • Maintain oversight of inventory and resources related to patient care.
  • Ensure accurate documentation of patient records, including data entry and processing.
  • Stay informed about Trust-held computer systems to facilitate efficient operations.
  • Participate in clinical and non-clinical audits to improve care quality.
  • Adhere to Trust standard operating procedures and policies.
  • Access qualified staff for guidance and support as needed.

General Duties

All employees are expected to comply with the following general duties:

  • Contribute to comprehensive needs assessments and the formulation of personalized care plans under the guidance of registered nursing staff.
  • Monitor and evaluate care delivery outcomes, reporting any changes in patient conditions to the registered nurse.
  • Conduct patient reviews on the ward as directed by the registered nurse.
  • Recognize and respond appropriately to emergencies, ensuring timely alerts to the healthcare team.
  • Manage incoming calls, providing initial triage to patients and caregivers.
  • Address inquiries and determine the best solutions, collaborating with Clinical Nurse Specialists as needed.
  • Offer general advice and direct patients and caregivers to additional resources, including healthcare professionals and informational materials.
  • Coordinate appointments and investigations to enhance patient experience.
  • Work with the MDT coordinator to ensure seamless transitions through the cancer care pathway.
  • Identify and address informational needs of patients and caregivers, connecting them with support services.
  • Support the registered nurse in facilitating safe transitions between services.
  • Deliver educational support to patients and caregivers regarding self-care management.
  • Encourage healthy lifestyle choices among patients.
  • Provide Level 1 psychological support, referring to registered nurses for Level 2 support when necessary.
  • Conduct follow-up calls to assess patient well-being and emerging needs, referring to specialists as appropriate.

Qualifications and Experience

Essential:

  • NVQ Level 3 or equivalent experience.
  • Previous experience in an acute hospital setting.

Desirable:

  • Experience with PICC line care and peripheral blood sampling.

Skills and Attributes

Essential:

  • Proficient in Microsoft Outlook, Word, and Internet Explorer.
  • Ability to work independently and manage emotional situations effectively.
  • Strong prioritization skills and ability to work under pressure.
  • Flexible approach to work and adaptability to changing circumstances.
