About the Role

We are seeking a highly skilled and compassionate Cancer Care Coordinator to join our team at Self Help Nottingham. As a key member of our Primary Care Network, you will play a vital role in providing holistic support to patients on the cancer pathway.

Key Responsibilities
  • Conduct in-person Cancer Care Reviews and liaise with a range of professionals to address the wider wellbeing needs of patients.
  • Assess and support patients with complex needs, ensuring they receive comprehensive care and support.
  • Work closely with GP surgeries, community organizations, and external services to ensure seamless care coordination.
  • Develop and implement targeted interventions to improve cancer screening rates and awareness within the community.
  • Collaborate with the Macmillan Beyond Diagnosis Service to provide holistic support to patients and their families.
Requirements
  • Experience of working in a healthcare setting, preferably in a cancer care or support role.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a high level of emotional resilience and empathy.
  • Knowledge of cancer and its impact on patients and their families, as well as relevant legislation and practices.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
