Job Overview

We are seeking a highly skilled and experienced Lead Nurse to join our Older Adult In-Reach and Home Treatment Team. As a key member of our multidisciplinary team, you will be responsible for providing clinical leadership and expertise in the delivery of high-quality care to individuals with functional mental health illnesses.

Main Responsibilities
  • Provide clinical leadership and expertise in the delivery of care to individuals with functional mental health illnesses.
  • Work collaboratively with colleagues, key partners, and community stakeholders to develop and implement effective care plans.
  • Employ a wide range of skills and expertise to support individuals in their own homes, preventing hospital admissions where possible.
  • Develop and maintain strong relationships with community mental health teams, GPs, and social care services.
  • Provide supervision and mentorship to team members and students.
Requirements
  • Recent and relevant post-qualification experience working with functional older adults.
  • Nursing degree/diploma and NMC registration.
  • Mentorship/SSSA trained.
  • Supervision trained.
  • Knowledge of MHA/Capacity Act/Legal frameworks.
  • Able to meet the travel requirements of this post.
  • Aware of own limitations.
  • Good clinical assessment skills.
  • Good demonstration of risk assessment skills.
  • Knowledge of functional illness.
