Role Overview:

  • Lead and coordinate a solution-focused strategy, engaging individuals in suitable services to transition them away from homelessness.
  • Identify and connect with individuals experiencing homelessness and those in need, providing immediate assistance through outreach efforts at various times and locations, including those hesitant to engage with support services.
  • Manage a caseload of individuals with intricate needs, crafting tailored action plans aimed at reducing instances of homelessness.
  • Engage in collaborative initiatives with multiple agencies to mitigate street activity.
  • Collaborate closely with substance misuse and addiction teams to facilitate access to treatment for individuals struggling with addiction.
  • Proactively ensure that individuals have access to continuous healthcare services.
  • Contribute to the ongoing statistical evaluation of individuals experiencing homelessness and participate in necessary outreach counts.

Candidate Profile:

  • Proven experience in fields such as substance misuse, alcohol dependency, or mental health support.
  • Experience working with individuals facing multiple and complex challenges.
  • Knowledge of and experience in collaborative working with a track record of facilitating multi-agency meetings.
  • Strong communication, negotiation, and advocacy abilities.
  • Ability to establish and maintain relationships with a diverse range of services.
  • Capable of working independently as well as collaboratively within a team.
  • Proficient in IT, including the use of software applications such as MS Office.
  • Able to critically assess and reflect on interventions, service delivery, and areas for improvement.

Note: This position is not eligible for a Health and Care Worker visa. Candidates may qualify for sponsorship under the Skilled Worker route, provided they meet the minimum salary criteria for the role.
