Job Summary:
Children's Complex Care is seeking a highly skilled and motivated Clinical Case Manager to join our team. As a Clinical Case Manager, you will be responsible for managing a caseload of children and young people with complex healthcare needs, providing high-quality care and support to ensure the best possible outcomes.
Key Responsibilities:
- Assess, plan, implement, and evaluate evidence-based, family-centered care for children and young people with complex healthcare needs.
- Collaborate with professionals across agencies and sectors to ensure safe and effective service delivery.
- Provide essential training and support to staff working in packages to ensure safe care delivery through our in-house Education Pathway.
- Manage a clinical caseload in the community setting, completing assessments and bespoke care delivery plans.
- Contribute to Clinical Governance and Risk Management, ensuring nursing practice is in accordance with the NMC Codes of Conduct.
- Work as part of the Education Team, providing classroom-based training to community staff, educating, supporting, and completing competency assessments for non-registered healthcare staff.
- Carry out staff appraisals and supervisions for registered nurses and Healthcare Assistants.
Requirements:
- UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
- Experience in managing complex healthcare needs and providing high-quality care and support.
- Ability to work collaboratively with professionals across agencies and sectors.
- Strong training and education skills, with experience in providing classroom-based training.
- Excellent communication and interpersonal skills, with the ability to work effectively with children, young people, and their families.
