About the Role

Rehabilitation Case Manager

This is a home-based role with travel to visit patients within your region. You will be working with a caseload of patients, assessing their clinical needs and coordinating their treatment. This role offers excellent work-life balance and flexibility.

Key Responsibilities

  1. Conduct thorough assessments of patients to determine their clinical needs and maximise recovery outcomes.
  2. Work with other stakeholders, such as solicitors and insurers, to access funding for private and augmentative intervention.
  3. Independently manage a caseload of patients and maintain deadlines for reporting and other administrative elements of each case.

Requirements

Essential Criteria:

  1. UK recognised qualification in Nursing, Physiotherapy, Occupational Therapy, Clinical Psychology or Sports Rehabilitation.
  2. Active professional board registration and public liability insurance (HCPC, NMC).
  3. Five years of clinical/practitioner experience.
  4. An understanding of statutory services provisions in relation to life-changing injuries.

Desirable Criteria:

  1. Clinical experience with a specialist focus on either brain injury/neurorehabilitation, spinal injury or paediatrics.
  2. Experience in the field of rehabilitation case management OR experience of the personal injury market highly preferred.
