Job Title: Clinical Case Manager

We are seeking a highly skilled and motivated Clinical Case Manager to join our team at Childrens Complex Care. As a Clinical Case Manager, you will be responsible for managing a caseload of children and young people with complex healthcare needs, providing high-quality care and support to ensure the best possible outcomes.

Key Responsibilities:
  • Assess and develop individualized care plans to meet the unique needs of each child or young person.
  • Collaborate with multidisciplinary teams to ensure safe and effective care delivery.
  • Provide education and training to staff on clinical delegated tasks and ensure competency assessments are completed.
  • Conduct staff appraisals and supervisions to ensure nursing practice is in accordance with the NMC Codes of Conduct.
  • Ensure clinical governance and risk management practices are in place to deliver safe and effective care.
Requirements:
  • UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
  • Experience working with children and young people with complex healthcare needs.
  • Ability to work autonomously and as part of a team.
  • Strong communication and interpersonal skills.
