Job Title: Clinical Case Manager

Childrens Complex Care is seeking a highly skilled and motivated Clinical Case Manager to join our team. As a Clinical Case Manager, you will be responsible for managing a caseload of children and young people with complex healthcare needs, providing high-quality care and support to ensure the best possible outcomes.

Key Responsibilities:
  • Assess and develop individualized care plans to meet the unique needs of each child or young person
  • Collaborate with healthcare professionals, families, and other stakeholders to ensure safe and effective care delivery
  • Provide education and training to staff on clinical delegated tasks and ensure competency assessments are completed
  • Conduct staff appraisals and supervisions to ensure nursing practice is in accordance with NMC Codes of Conduct
  • Ensure clinical governance and risk management practices are in place to deliver safe and effective care
Requirements:
  • UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills
  • Experience working with children and young people with complex healthcare needs
  • Ability to work collaboratively with professionals across agencies and sectors
  • Strong commitment to quality and high standards of care and service delivery
