Clinical Case Manager - Adult & Paediatrics

We're seeking a skilled Clinical Case Manager to join our Case Management Team at HCA Healthcare UK. As a Clinical Case Manager, you will work closely with healthcare professionals to assess, plan, implement, and evaluate care options and services that meet patients' health needs.

Key Responsibilities:

  1. Assess patients' health needs and develop comprehensive care plans
  2. Collaborate with healthcare professionals to coordinate proactive discharge planning activities
  3. Act as a liaison between patients, hospitals, physicians, and third-party payers to optimize community treatment and services

Requirements:

  1. NMC Registered Nurse with substantial postgraduate experience (Adult registered with Paediatric experience)
  2. Experience of utilisation review and extensive discharge planning experience essential
  3. Experience as a Senior Staff Nurse/ Band 6 or above essential
  4. Paediatric experience
  5. Extensive knowledge of discharge planning and community resources
  6. Possess ability to take initiative to work independently and within teams
  7. Possess excellent communication, organisation, and computer skills and have a dynamic approach to problem solving
