Job Summary:

The Clinical Case Manager will be a driven, motivated UK registered nurse (RSCN or RGN), with strong clinical, management and communication skills to safely manage a caseload of children and young people with complex healthcare needs across our growing service.

Key Responsibilities:

  • Clinical Case Management: Safe and effective management of a clinical case load in the community setting;
  • Assessment and Planning: Complete assessments and bespoke care delivery plans.
  • Clinical Governance and Risk Management:
  • Education and Training: Work as part of the Education Team in providing classroom based one to one/ bespoke training to community staff where required;
  • Staff Development: Educate, support and complete competency assessments for non-registered healthcare staff on clinical delegated tasks;
  • Leadership and Management: Carry out staff appraisals and supervisions for registered nurses and Healthcare Assistants;
  • Collaboration and Partnership: Effective engagement and collaborative working with ICB's and external MDT;
  • Nursing Practice: To ensure nursing practice is in accordance with the NMC Codes of Conducts;
  • Clinical Accountability: Clinically accountable and responsible to ensure all reasonable measures are in place to deliver safe effective evidence- based care to all children/ young people in collaboration with all colleagues including on call and Bookings Team, to the best of yours and the organisation's capabilities

Requirements:

  • UK registered nurse (RSCN or RGN)
  • Strong clinical, management and communication skills
  • Ability to work in a community setting
  • Experience in clinical case management
  • Ability to work collaboratively with ICB's and external MDT
