We are seeking an experienced and highly motivated Project Worker Complex Needs Specialist to join our team at Single Homeless Project. The successful candidate will work alongside the Regional Services Manager to provide a high level of support to clients with complex needs at our Ashley Road and Milton House projects.
Key Responsibilities- Provide specialist support to clients with complex needs, including mental health, physical health, and substance use issues.
- Work closely with external professionals to reduce the risk of re-offending and prevent homelessness.
- Develop and implement individualized support plans to promote independence and social inclusion.
- Manage a caseload and deliver positive outcomes, utilizing knowledge of the health, social care, and criminal justice sector.
- Previous experience of working with people with complex support needs.
- Ability to work in a flexible and innovative way with a challenging client group.
- Strong communication and interpersonal skills.
- Knowledge of the health, social care, and criminal justice sector.
