Position Overview

The Enhanced Care Support Coordinator is responsible for providing daily guidance and support in the management of patients who require specialized care plans. This role ensures that the healthcare assistants are functioning effectively and that the team is delivering a high-quality, responsive service that prioritizes patient satisfaction.

Key Responsibilities

• Collaborate closely with ward managers, matrons, and specialist teams (including the Psychiatric Liaison Team, Dementia Team, and Learning Disability Liaison) to facilitate the delivery of appropriate therapeutic support within the ward environment.

• Assist in the ongoing assessment of patients who may require additional behavioral management strategies due to transient or permanent conditions.

• Engage in care planning for patients necessitating enhanced care responses, working in conjunction with multidisciplinary teams.

• Promote therapeutic relationships that respect patients' cultural values, beliefs, and preferences, ensuring dignity and independence.

• Support staff in conducting continuous risk assessments of patients.

• Foster effective communication within the immediate care team and the broader multidisciplinary team, ensuring a cohesive approach to patient care.

• Enable the provision of evidence-based assessments and treatments that consider cultural preferences and health beliefs.

• Facilitate the delivery of specialized mental health education to Trust staff, enhancing awareness and empowering professionals to improve the service user experience.
