Position Overview

Key Responsibilities

The successful candidate will be responsible for:

  • Monitoring and managing patients on a suspected cancer pathway to ensure timely diagnosis within the established standards.
  • Coordinating appointments and follow-ups for Two-week rule clinics, including making courtesy calls to patients.
  • Identifying and reporting any deviations in patient pathways using established local protocols.
  • Collaborating with the Colorectal and broader clinical teams to enhance service delivery and meet the required standards.

Daily Duties

The role includes:

  • Working closely with multidisciplinary teams, including Cancer Trackers and Clinical Offices, to ensure seamless patient management throughout the cancer pathway.
  • Monitoring patient progress from referral through diagnostics and treatment, addressing any potential delays proactively.
  • Ensuring accurate data collection to facilitate timely patient management and identify avoidable delays.
  • Liaising with various departments to ensure all elements of patient pathways are coordinated effectively.
  • Providing up-to-date information to the multidisciplinary team following cancer diagnosis to ensure adherence to national treatment targets.

Communication and Engagement

The role requires excellent communication skills to:

  • Engage with patients and their families, providing updates and support throughout their treatment journey.
  • Maintain effective communication with clinical teams, GPs, and external agencies while ensuring patient confidentiality.
  • Conduct training sessions for staff regarding cancer standards and the role of the pathway coordinator.

Professional Development

The position offers opportunities for personal growth, including:

  • Participation in the induction of new staff and ongoing education about the pathway coordinator's role.
  • Engaging in performance reviews and pursuing further training relevant to the position.
