Pure Care Recruitment is seeking a motivated and dedicated Care Coordination Specialist to join our dynamic team within a well-established home care organization.

This role is primarily office-based, operating from Monday to Friday, with standard working hours.

Key Responsibilities:

  • Developing and revising individualized care plans to ensure tailored support for clients.
  • Conducting regular supervisions and performance evaluations of care personnel.
  • Managing incoming calls professionally and addressing inquiries effectively.
  • Assisting the Registered Manager in fostering a cohesive and skilled workforce, including recruitment and training initiatives.

Qualifications:

The ideal candidate will possess at least an NVQ Level 3 in Health and Social Care and have prior experience in a supervisory or managerial capacity within a care environment, whether in home care or residential settings.

While previous experience as a Care Coordinator or Office Manager is advantageous, we value a positive attitude and a willingness to learn and enhance existing skills.

Compensation:

The starting salary for this position is competitive, with additional financial incentives available for on-call responsibilities, which are shared among the management team.
