The Position

Superior Healthcare is looking for a Clinical Care Manager for our Complex Care Division in Kent. With a focus on delivering over 650,000 hours of care annually, the Clinical Care Manager will collaborate with a team of professionals. In this role, you will manage a caseload of clients requiring complex care, fostering strong relationships with them and their families, overseeing their care plans, coordinating with commissioners and case managers, and supporting the onboarding of new staff. The primary objective of a Clinical Care Manager is to ensure that clients receive outstanding service from a well-trained, dedicated team of Healthcare Support Workers and Registered Nurses.

Candidate Profile

  • You must be an Adult trained Registered Nurse with a valid NMC PIN.
  • A minimum of three years' post-registration experience is required.
  • Experience in delivering community-based care, particularly complex care, is essential, along with proficiency in PEG, tracheostomy care, and ventilators.
  • You should have the right to work in the UK and possess a clean UK driving license.

Key Duties

  • Develop personalized care plans and provide guidance.
  • Conduct staff supervision, audits, and case management responsibilities.
  • Participate in the 24-hour service team, taking on nursing shifts as needed.
  • Lead care packages and support the Care & Nursing team.
  • Location: We are recruiting across various areas within Kent, covering specific regions.
  • You should be adaptable to the needs of the service and clients, demonstrating commitment and a drive to act in the clients' best interests.
