Position Overview

The Patient Care Liaison plays a vital non-clinical role within the Medway Rainham Primary Care Network (PCN).

This position offers a unique opportunity to gain insights into all PCN services while collaborating with a diverse range of stakeholders, including PCN personnel, healthcare providers, patients, and community organizations, serving as a key point of contact.

Key Responsibilities

  • Serve as the initial contact for patients and healthcare professionals in managing patient care.
  • Foster engagement by encouraging patients, their families, and caregivers to provide feedback and actively participate in the care coordination process.
  • Ensure prompt follow-up and action for patients based on communications from community and secondary healthcare services.
  • Oversee tasks to guarantee their completion and the delivery of care through regular audits of the clinical system.
  • Direct and arrange appointments, follow-ups, or other necessary actions to assist the PCN in delivering high-quality, compassionate care to our patient community.
  • Work empathetically with patients, their families, and caregivers to gather essential information, ensuring comprehensive and accurate records of support.
  • This role also provides opportunities to contribute to broader PCN initiatives.
  • The PCN may require additional support as needed, including covering for colleagues during their absence.

About Our Organization

We are a collaborative network of GP practices that have been working together since July 2019.

Our mission is to share best practices and implement meaningful changes collectively to enhance the care we provide to our patients.

This organization aims to coordinate activities around patient groups requiring ongoing care while continuing to deliver standard GP services.

We aspire to collaborate with a broader range of authorized providers when necessary to improve patient care pathways.

Job Duties

The Patient Care Liaison collaborates closely with GP practices and staff within the PCN to identify and manage a caseload of patients, ensuring that appropriate support is available to them and their caregivers while addressing their evolving needs.

This role is crucial in facilitating enhanced continuity of care by acting as a reliable point of contact for residents, families, and healthcare professionals.

Required Qualifications

Essential

  • Experience in a general practice setting.
  • Familiarity with EMIS software.

Desirable

  • Experience with systems such as Apex, Ardens Manager, Docman, and GPAD.
