Secondment Opportunity - Community Nursing Team Supervisor Band 7 (Qualified Nursing Team Supervisor)

An exceptional opportunity has emerged for qualified Community Nurses with pertinent experience in community healthcare to become part of our Community Nursing Teams.

Our innovative Community Nursing teams are dedicated to providing exceptional patient care. As a Community Nursing Team Supervisor, you will play a crucial role in guiding the delivery and enhancement of our services. This position is well-suited for an experienced Community Nurse or a newly qualified Specialist Community Nurse eager to advance their leadership capabilities and gain relevant experience in community healthcare.

The ideal candidate will possess substantial experience in nursing at a Band 6 level or higher, with a solid understanding of community healthcare practices being essential.

Main Responsibilities

The post holder will offer management support and professional nursing leadership to all members of the nursing team. They will collaborate with Senior Nurses to develop, implement, and assess new initiatives related to Community Nursing services and inspire staff to facilitate necessary changes.

The post holder will manage a clinical caseload, which will be GP attached. This caseload will include housebound patients, with additional sites designated for commissioned wound clinics. The post holder will be accountable for all staff within their designated team, where clinical and managerial support is vital for team and service advancement. The role also encompasses change management, where the Community Nursing Team Supervisor will assist their team in positively embedding changes.

As a Team Supervisor, you will operate within an integrated service that includes specialized care teams such as palliative care, tissue viability, bladder and bowel management, cardiac care, neurology, diabetes, rapid response, and specialized therapy teams like physiotherapy and occupational therapy. Our Community Nursing service is also supported by an evening Twilight team. All community services strive to collaborate to ensure individualized, patient-centered care alongside support from our GP colleagues and practices.

Job Responsibilities

As one of the leaders in Community Nursing, in collaboration with other locality team members, the post holder will facilitate all aspects of care and information for patients and assist staff already involved in patient care through the following interventions:

  • Serve as a role model in expert practice, facilitating and supporting effective patient care through clinical judgment and decision-making skills.
  • Ensure Community Nursing teams develop appropriate care plans for individual patients, families, or patient groups.
  • Take ongoing responsibility for assessing care needs and developing, implementing, and evaluating care programs for individuals and their families.
  • Lead the creation of health profiles, establish objectives, and prioritize within the caseload.
  • Innovate methods for identifying patients, particularly focusing on housebound individuals and the elderly, from community locations, providing assessments and care programs to meet their primary health needs.
  • Maintain a visible professional presence where staff, patients, caregivers, and services can seek assistance, advice, and support to enable holistic care.
  • Implement the framework of informed consent.
  • Utilize advanced specialist knowledge to empower patients and caregivers to develop self-care skills regarding health and healthcare services.
  • Ensure that necessary supplies and equipment are available and maintained according to best practices in client care, reviewing as per local policy.
  • Document consultations and practices to incorporate both audit and the setting, monitoring, and evaluation of care standards.
  • Act as a key coordinator to promote seamless service delivery.
  • Lead and advocate for practice development areas, devising guidelines and procedures to support care delivery.
  • Implement National Service Frameworks.
  • Ensure nursing care is evidence-based and adheres to best practices.
  • Lead and facilitate appropriate audits to evaluate care programs and maintain high standards.
  • Participate in Trust-wide groups requiring specialist input.
  • Support care delivery through nurse prescribing.
  • Plan and manage chronic disease care, working to prevent complications of long-term conditions.
  • Contribute to health promotion initiatives.
  • Collaborate with training and development staff and other healthcare professionals to enhance the capacity of Primary Care Teams to address community health needs.
  • Work with partners to facilitate access to programs promoting community health (training, basic skills training, adult education).

Qualifications and Experience

Essential

  • Registered General Nurse (RGN)
  • Educated to Degree Level or above
  • Degree qualifications in Public Health, Community Health, or Specialist Practitioner
  • Mentorship qualification

Desirable

  • Management course - Certificate in Management Studies (CMS)
  • Teaching Qualification - working towards
