Nurse in Community Care

We are seeking a highly skilled and motivated Nurse to join our team at Central London Community Healthcare NHS Trust. The successful candidate will be responsible for providing high-quality care to patients in the community, working closely with our multi-disciplinary team to deliver excellent patient outcomes.
Main Responsibilities- Provide comprehensive nursing care to patients in the community, including assessment, planning, implementation, and evaluation of care.
- Work collaboratively with our multi-disciplinary team to deliver high-quality patient care, including social services, GPs, district nursing, and voluntary services.
- Develop and implement evidence-based care plans, ensuring that patients receive the best possible care and support.
- Supervise and mentor junior staff, providing guidance and support to ensure that they are equipped to provide high-quality care.
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
- RGN/RN with current registration with the NMC.
- Evidence of recent and continuing post-registration professional education and training.
- Practice supervisor/assessor training.
- Experience of working in a community setting, with a focus on delivering high-quality care to patients with complex health needs.
- Ability to work autonomously, prioritizing and managing own workload, as well as that of others.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
