We are seeking a highly skilled and experienced Senior Adult Nurse Case Manager to join our team at Leaders in Care, a well-established provider of complex care packages within the community.
Key Responsibilities- Manage a reduced caseload of service users, providing personalized care plans and delivering high-quality nursing interventions.
- Support the growth and development of our team by mentoring Adult Nurse Case Managers and contributing to the design and delivery of tailored care plans.
- Effectively communicate and collaborate with multidisciplinary teams, including community teams, CHC nurses, and CCG commissioners, to establish and maintain bespoke packages of care.
- Build and maintain strong relationships with clients and their families, providing ongoing monitoring, support, and assessment of their physical and physiological needs.
- Deliver nursing interventions, including the management of medicine delivery, and work closely with carers to provide face-to-face support and training.
- RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators within the last 2 years.
- Advanced communication skills, both verbally and in writing, to liaise effectively with patients, families, and multidisciplinary teams.
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs.
- Full UK Driving Licence desirable but not essential.
- Up to £50,000 per annum DOE.
- 1 day working from home per week / 4 days in the community.
- Monday to Friday (9-5) hours.
- 33 days holiday.
