Adult Clinical Nurse Advisor

We are seeking a highly skilled and experienced Adult Clinical Nurse Advisor to join our team at Leaders in Care. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, creating personalized care plans, and working closely with a core team of carers to deliver high-quality care.

Key Responsibilities:
  • Develop and implement effective care plans that meet the unique needs of each client
  • Provide expert clinical guidance and support to carers and other healthcare professionals
  • Conduct regular care rounds and proactive home visits to ensure the highest level of care
  • Respond to and investigate clinical complaints, in line with policy and risk management
  • Collaborate with the multidisciplinary team to ensure seamless care delivery
Requirements:
  • RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators
  • Advanced communication skills and ability to train clinical and non-clinical staff
  • Full UK Driving Licence essential for community work
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management
